Medical Requirements
Two medical forms are required for each child prior to
commencing school.
The Erasmus School Medical Form, obtainable from the
school office, provides a medical background including allergies,
special needs, Doctor’s details, etc. This enables the School to
provide the best possible care for each child.
The Immunisation Status Certificate provides details of
the child’s current protection from disease. This can be supplied
in either of two forms. If the Immunisation History Statement
includes the words, ‘This child has received all vaccines required
by 5 years of age’, a copy of this is sufficient.
If the immunisation is not complete, the local Council will
provide an interim certificate.
Should parents prefer not have the child immunised, a Statutory
Declaration to this effect can be taken to the Local Council’s
Health Department for a suitable Status Certificate to be provided.
It is a legal requirement the either the Status Certificate or the
Immunisation History Statement must be lodged with the school
office prior to the start of the child’s first term.
